Children’s and Adolescents’ Diary (2 weeks)

The Sleep Diary for Children and Adolescents is a simple and easy-to-follow tool that helps track sleep, wakefulness, and daily habits over a 2-week period. Regular completion makes it easier to understand the child’s or teenager’s daily rhythm, sleep quality, and the factors that may affect their rest.

Children’s and Adolescents’ Diary (2 weeks)

The diary allows users to record:

  • bedtime and sleep onset periods,
  • whether the day is a school day or a day off,
  • physical activity (“S” – sports),
  • caffeine intake (“K” – coffee, tea, cola, energy drinks, chocolate),
  • medication use (“Z”),
  • as well as comments about wellbeing, illness, or special circumstances.

 

This diary helps the child and parents, as well as the doctor or specialist, to better assess sleep habits, the causes of tiredness, and the need for further recommendations or investigations.

 

Note: If the diary is not completed on a particular day, it can simply be left blank – the main goal is to evaluate overall habits over a longer period of time.
